29 CFR  1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about June 24, 2021, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 3483 Grafton Road, Brunswick, Ohio 44212.  Employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.  Majia Roofing and Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1474016, citation number 1, item number 1 and was affirmed as a final order on 5/14/20, with respect to a workplace located at 2818 South Arlington, Akron, Ohio 44312.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about June 24, 2021, employees were exposed to a fall hazard of approximately 10 feet while performing roofing activities on a residential home located at 3483 Grafton Road, Brunswick, Ohio 44212.  Employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.  Majia Roofing and Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1474016, citation number 2, item number 1 and was affirmed as a final order on 5/14/20, with respect to a workplace located at 2818 South Arlington, Akron, Ohio 44312.   Majia Roofing and Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1326767, citation number 1, item number 1 and was affirmed as a final order on 7/16/18, with respect to a workplace located at 2756 Stony Hill, Medina, Ohio 44256.
