29 CFR  1910.67(c)(2)(v): A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift:   On or about June 16th, 2021 at 446  South Beach Road Hobe Sound, FL 33455, an employee was exposed to fall hazards up to 15 feet when operated an aerial lift while not wearing harnesses with lanyards connected to anchor points and/or the d-rings of the harnesses.
