29 CFR 1910.67(c)(2)(v):(v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift. a) Main Production Building- On or about June 10, 2021, an employee working from the Skyjack Boom Lift was not wearing a body belt or full body harness. The employee was thereby exposed to the hazards associated with a fall of approximately eight (8') feet.

29 CFR 1910.147(c)(8):Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: a) Main Production Building- On or about June 10, 2021, the employer failed to ensure the authorized employee, assigned the task of troubleshooting the burner on the Rotary Furnace # 3, performed lockout prior to servicing and/or maintaining the equipment, thereby exposing the employee to crushing and/or struck-by hazards.
