Philadelphia, PA - 19113: The employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2021. An employee for the company was struck by a garage door and fell approx. 30ft. to ground level from a scissor lift, thus sustaining severe injuries. Condition was noted on or about June 28, 2021.
