On June 16, 2021 at 7020 Packer Drive in Wausau, Wisconsin, the employer did not ensure the Hyundai forklift operator had a current performance evaluation within three years. The forklift was used for placing and removing products and materials on storage racks in the warehouse.

29 CFR 1926.50(c): In the absence of an infirmary, clinic, hospital, or physician, that is reasonably accessible in terms of time and distance to the worksite, which is available for the treatment of injured employees, a person who has a valid certificate in first-aid training from the U.S. Bureau of Mines, the American Red Cross, or equivalent training that can be verified by documentary evidence, shall be available at the worksite to render first aid. On June 16, 2021 at 6937 US 51 in Hazelhurst, Wisconsin, the employer failed to provide prompt certified first-aid for an employee who suffered an amputation injury on a worksite located seven miles from emergency medical services.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. The Employer failed to report an amputation injury to the Occupational Safety and Health Administration (OSHA) within twenty-four hours of the incident. On June 16, 2021 at 6937 US 51 in Hazelhurst, Wisconsin, an employee suffered a fingertip amputation during power saw operation; OSHA was notified of the injury on June 22, 2021.
