29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a) The employer failed to provide documentation demonstrating the abatement method and completion for no warning signs posted at entrances of rooms that contained asbestos issued on December 10, 2021 as Citation 1 Item 1.   (b) The employer failed to provide documentation demonstrating the abatement method and completion for not identified/labelled asbestos insulated piping in tunnels and hallways issued on December 10, 2021 as Citation 1 Item 1.

29 CFR  1910.1001(j)(4)(iv):  At the entrance to mechanical rooms/areas in which employees reasonably can be expected to enter and which contain ACM and/or PACM, the building owner did not post signs which identify the material which is present, its location, and appropriate work practices which, if followed, will ensure that ACM and/or PACM will not be disturbed:  a.  On or about June 24, 2021, entrances to boiler and equipment rooms that contained asbestos insulated piping and equipment were not posted with asbestos warning signs.  b.  On or about June 24, 2021,  asbestos insulated piping in tunnels and hallways was not identified/labelled to warn employees of asbestos containing material.

29 CFR  1910.22(a)(2): The employer did not ensure that the  floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition.   a)  On or about June 24, 2021, in the Training Center and the PK Lab and other areas, floors were not maintained in a dry and clean condition, in that floors were not cleaned up during roof leaks or have effect catch buckets in place to prevent water from splashing onto the floor.  This exposed employees to a slip and fall hazard.

29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a) On or about June 24, 2021,  a written respiratory protection program with worksite-specific procedures was not established and implemented.  Maintenance employee(s) used a 3M half face elastomeric respirator during tasks that generate dust, such as the changing of filters in the air handling systems.
