The employer requires employees to use filtering facepiece respirators to protect against exposure to SARS-CoV-2 in the workplace.

Employees working as home health aids were not provided with the required site-specific information, such as adequate workplace-specific policies and procedures.

Names of direct care workers who tested positive for COVID-19 were listed in the notes section of a spreadsheet pertaining to consumer exposure to COVID-19.
