29 CFR  1926.451(f)(3):Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  On June 24, 2021, located near an entrance door to a commercial establishment and an active parking lot of job-site address 940 S. Kenwood Rd., Suite D., Blue Ash, OH 45242. Employees were exposed to a fall hazard working from a scaffold system as there were no inspections performed by a competent person before the work shift to ensure employees were protected from falling by installing a personal fall arrest system, or guardrail systems. Potential fall height measured to be in excess of 10 feet.

29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.  On June 24, 2021, located near an entrance door to a commercial establishment and an active parking lot of job-site address 940 S. Kenwood Rd., Suite D., Blue Ash, OH 45242.  Employees were exposed to a fall hazard while repairing windows in a roof skylight while working from a scaffold system and not utilizing a personal fall arrest system, or guardrail system. Potential fall height measured to be in excess of 10 feet.

29 CFR  1926.454(a):The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:  a) The nature of any electrical hazards, fall hazards, and falling object hazards in the work area;    b) The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used;     c) The proper use of the scaffold and the proper handling of materials on the scaffold;      d) The maximum intended load and the load-carrying capacities of the scaffolds used; and      e) Any other pertinent requirements of this subpart.  On June 24, 2021, located near an entrance door to a commercial establishment and an active parking lot of job-site address 940 S. Kenwood Rd., Suite D., Blue Ash, OH 45242. An employee was working from a scaffold system had not received training from a qualified person to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Potential fall height measured to be in excess of 10 feet.
