29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees. Bayer Crete Inc., on or about June 24, 2021 at the address of 449 North Hermitage rd., Hermitage, PA: Employees worked from a height of roughly 20 feet without the use of a top rail for employees to utilize as a means of fall protection during work operations from the scaffolding platform.
