At worksite located at 6801 Lot 19 Meadows Creekside, Parkville MO -- employee engaged in framing duties using a portable electric skilsaw and was not wearing eye protection. This exposed employees to struck by hazards in that debris could get in his eyes. This was documented on or about 06/28/2021.

This exposed both to a fall hazard greater than 12 feet to ground level. This was documented on or about 06/28/2021.
