29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At worksite located at 6801 Lot 19 Meadows Creekside, Parkville MO -- employee engaged in framing duties using a portable electric skilsaw and  was not wearing eye protection. This exposed employees to struck by hazards in that debris could get in his eyes. This was documented on or about 06/28/2021.

29 CFR  1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  At a residential worksite located at 6801 Lot 19 Meadows Creek, Parkville MO -- a platform used to raise material to the roof of the residential house was not an authorized approved platform specifically designed by the manufacturer for such use. This exposed the employee to struck by/fall hazards.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a worksite located at 6801 Lot 19 Meadows Creekside, Parkville MO -- employee engaged in framing duties was standing on a steep roof handing 4'X8' sheets of plywood to owner also on the steep roof without any kind of fall protection system. This exposed both to a fall hazard greater than 12 feet to ground level. This was documented on or about 06/28/2021.
