One of the 4 chains on the lifting device had a clevis link that was used in place of a chain link. The employer could not provide the rated capacity for the clevis link.

29 CFR 1926.251(b)(3): Job or shop hooks and links, or makeshift fasteners, formed from bolts, rods, etc. or other such attachments were used: a) Worksite - On or prior to June 24, 2021, a 4 way spreader chain was used to lift and lower materials including, but not limited to, catch basins. One of the 4 chains on the lifting device had a clevis link that was used in place of a chain link. The employer could not provide the rated capacity for the clevis link. Due to their being no information provided to OSHA regarding the rated capacity of the clevis link to ensure that the clevis link's rated capacity is at least equal to that of the alloy steel chain, the clevis link used on the chain sling is considered to be makeshift.
