29 CFR  1910.303(b)(1):Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  a) Kitchen - On or about 6/29/21,  the top outlet of the dual outlet, 120v receptacle (used to power for the printer, tablet, portable phone via a three outlet adapter) was damaged and missing a section potentially exposing live electrical. 29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Kitchen - On or about 6/29/21, the three outlet adapter used to power the printer (3 amperes), tablet ( 2 amperes), and portable phone ( 200 milliamperes).  The three outlet adapter was plugged into a dual outlet receptacle (operating at 120 volts).   The three outlet adapter was not used or installed in accordance with instructions included in the listing or labeling.  ABATEMENT CERTIFICATION REQUIRED
