29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Mechanic Street facility, Easthampton, MA: On June 29, 2021 and at times prior, employees who performed maintenance and servicing tasks did not utilize existing energy control procedures to prevent employee exposure to hazards. 1. Maintenance technicians entered the fence-guarded belt/pulley area during operation to check the condensation tank and during machine lubrication employees only turned off, but did not isolate and lock out the machine. 2. Employees who set up and threaded product onto the Pin Dryer machine did not utilize the existing energy control procedures. On June 10, 2021, an employee who was setting up the PIN dryer machine received arm injuries when another employee energized the machine.

29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: Mechanic Street facility, Easthampton, MA: On June 29, 2021 and for many months prior, the employer had not performed a comprehensive periodic inspection of the energy control procedure. The employer did not ensure each procedure was inspected for inaccuracies during an actual energy control task. Additionally, the employer did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under each energy control procedure they were authorized to use.
