OSH ACT of 1970 Section (5)(a)(1):Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to crushing and struck-by hazards: A. Between June 23, 2021 and November 12, 2021, at the employer's facility located at 3100 East Broadway, Alton, IL 62002, at the rear of the building, employees were exposed to a crushing hazard, due to active structural failure of the rear wall. B. Between June 23, 2021 and November 12, 2021, at the employer's facility located at 3100 East Broadway, Alton, IL 62002, employees were exposed to struck-by hazards, due to structural failure of the ceiling and roof, which caused metal sheets and beams to fall inside the building.
