29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a.) On or about June 18, 2021, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment, including Armor Inox Tank #3, to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Armor Inox Room - On or about June 18, 2021, an employee was exposed to machine hazards associated with thermal energy while tightening the clover tri-clamp on Armor Inox Tank #3. The employer failed to implement energy control application steps as the machine as not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.
