29 CFR 1910.304(b)(3)(ii)(C)(3):Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug were not visually inspected before use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage, and equipment found damaged or defective was not removed from use until repaired: Location: shop area On or about 6/24/21, employees were exposed to electrical shock and burns when using extension cords without being inspected. The cords had damaged which included but not limited to missing ground pin, insulation missing and indications of cords being over stretched.
