29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about June 29, 2021, employees performing cleaning with acetone and grinding, blending, and welding operations of stainless steel, nickel, and cobalt were not provided with the effective information and training on hazardous chemicals in their work area. Employees were unaware of the chemical hazards they were exposed to while performing their duties and had not been provided adequate training on these exposures.

29 CFR 1910.252(a)(1)(i):When the object to be welded or cut could not be readily moved, all movable fire hazards in the vicinity were not taken to a safe place: Welding area: On or about June 29, 2021, flammable material was observed to be within 35 feet of the welding operation. Combustible material observed within a few feet around the point of operation included miscellaneous items such as water bottles, papers, clothing, and cardboard boxes where employees were welding.
