29 CFR 1910.178(m)(6): The employer did not ensure that safe distance was maintained from the edge of ramps or platforms while on any elevated dock, or platform or freight car: (a) On or about 06/29/2021 - Dock door #16 area, NISSAN, Forklift, Model P4038, was being operated in close proximity to the edge of the dock platform, exposing employees to crushed-by hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws: (a) On or about 06/29/2021 - Dock door #18, the 208-Volt electrical junction box conduit did not have a strain relief connector exposing employees to electrical hazards.
