On and about 6/30/2021 employees were exposed to electrical hazards where a flexible cord was connected to a power source which was energized from another flexible cord being used as a substitute for fixed wiring.

29 CFR 1910.176(e): Clearance signs to warn of clearance limits were not provided where mechanical handling equipment was used: 400 West Clay, St. Charles, MO 63031: Employer is exposing employees to hazardous overhead conditions where the employees are permitted to store items on racks without limiting the storage clearances where mechanical handling equipment is permitted. On and about 6/30/2021 employees were exposed to overhead falling hazards where a clearance limit sign was not posted to inform the employees of the clearance limits for storage on aisles such as, but not limited to, 29, 30, and 32 where products were stored with no discernable spaces between overhead beams or sprinkler heads and the products stored on the top shelves.
