29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  Warehouse: Employer is exposing employees to hazardous conditions where the employee are permitted to work where unmounted portable fire extinguishers are left on workplace floor.   On and about 6/29/2021, north of the warehouse, an unmounted portable fire extinguisher was left on workplace floor by the electrical power installation equipment.  Another unmounted portable fire extinguisher was left on the workplace floor by a door used for accessing front section of the establishment. The employees were exposed to tripping hazards.

29 CFR  1910.176(a): Permanent aisles or passageways were not appropriately marked:  Warehouse: Employer is exposing employees to struck-by hazards where the employees are permitted to walk  in the warehouse where permanent aisles are not marked. On and about 6/29/2021 employees were exposed to struck-by hazards where a mechanical material handling piece of equipment was used without markings of the permanent aisles.

29 CFR  1910.176(b): Storage of material created a hazard:  Warehouse: Employer is exposing employees to hazardous conditions.  The employees are exposed to overhead falling objects where items are stored on damaged storage racks. On and about 6/29/2021 the employees pulled items from and stored items on storage systems which had twisted uprights from damage, bent horizontal members, and rusted members.  The employees were exposed to overhead falling objects where the storage of materials created hazards on racks 2, 3, 5, 6, 7, 26, 27, and 29.

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  Warehouse: Employer is exposing employees to hazardous conditions where operators of forklifts are not provided required training. On and about 6/29/2021 employees who operated forklifts were not officially trained to operate a Clark forklift in use at the establishment.

29 CFR  1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:  Warehouse: Employer is exposing employees to amputation hazards where the employees are permitted to operate a pedestal fan which has guard openings which are larger than 1/2 inch. On and about 6/29/2021 the employees used a potable pedestal fan which had guard openings which were measured at approximately 1 inch.

29 CFR  1910.332(b)(3): Employees were not trained in and familiar with the safety related work practices required by 29 CFR 1910.331 through 29 CFR 1910.335 that pertained to their respective job assignments:   Warehouse: Employer is exposing employees to electrical hazards where the employees performing electrical work activities are not provided required training. On and about 6/29/2021 an employee replacing electric ballasts with new ones received a massive electric shock.  The employee was not provided the required safety related work practices training.
