29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Building Exterior: Employees were exposed to eye/face injuries while cutting bricks with a masonry trowel without the use of eye and face protection. Employees were not protected with any type of eye/face protection, on or about 6/24/21.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    a) Building Exterior: Employees were using pipe scaffold frames and cross braces to access the scaffold, exposing employees to a fall of approximately 10 feet to the ground surface, on or about 6/24/21

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) Building Exterior: Employees were exposed to a fall hazard of approximately 10 feet while walking/working on the scaffold platform without the use of personal fall arrest system or other means of fall protection in place, on or about 6/24/21.
