29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports as follows:  a. At this job site, located at 1550 Tynes Blvd, in Middleburg, Florida: On or about July 1, 2021, the employer exposed employees to fall hazards in that, the three buck high scaffold system was not fully planked.  It had one plank on the outrigger where the employees were washing the wall and being exposed up to a 22-foot fall hazard.

29 CFR  1926.451(f)(7):Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  a. At this job site located at 1550 Tynes Blvd, Middleburg, Florida: On or about July 1, 2021,  the employer exposed employees to fall and crushing hazards in that,  employees dismantled, moved and erected the three buck high scaffold system without the supervision of a competent person being at the job site.

29 CFR  1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.    a. At this job site, located at 1550 Tynes Blvd, in Middleburg, Florida: On or about July 1, 2021, the employer exposed employees to fall hazards in that, the 3-buck high scaffold system did not have a ladder exposing employees performing brick washing operations to a 22 foot fall hazard.  Reyes Brothers Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1) , which was contained in OSHA inspection number 1431978, citation number 01,  item number 001 and was affirmed as a final order on March 11, 2020, with respect to a workplace located at 425 Harrison Ave, New Orleans, LA 70124.
