29 CFR  1926.501(a)(2):The employer did not determine if the working/walking surfaces on which its employees were to work had the strength and structural integrity to support the employees safely.  On or about June 24, 2021, the employer did not determine if the working/walking surface had the strength and structural integrity to support two employees who were installing safety lines on a commercial roof under renovation, exposing the employees to a fall hazard.

29 CFR  1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   On or about June 24, 2021, two employees suffered injuries that resulted in hospitalizations. The hospitalizations were not reported within 24 hours to the local Area Office of the Occupational Safety and Health Administration.
