The employees were exposed to fall hazards over 25 feet to lower levels and the ground below.    Note) Certification of corrective action is required for this item.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) On or about June 30, 2021; employees used a mobile scaffold as part of a set up to access their work area on the roof from the interior of the building.    The mobile scaffold did not have guardrails in place, exposing employees to a 16 foot fall to the floor below.      Note) Certification of corrective action is required for this item.
