29 CFR 1926.961(c)(13):  The employer did not ensure that no one initiates action to reenergize the lines or equipment at a point of disconnection until all protective grounds have been removed, all crews working on the lines or equipment release their clearances, all employees are clear of the lines and equipment, and all protective tags are removed from that point of disconnection:  (a) Jobsite, Clifford Ball Drive, Clinton, PA:  On or about June 26, 2021 employees were working in manhole 54818 splicing the K cable when the J cable was reenergized without their knowledge.  The J cable was positioned at head/neck level and within 2 feet of where the employees were working and they had not been cleared from the area when the reenergizing was done.
