29 CFR  1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    On or about June 28, 2021, the employer's lockout procedure for the Double End Tenorer (DET) did not specify the coast down time needed to delay the entry of the authorized employee to the hogger blades. The blades took 6 to 7 minutes to stop motion once the electrical energy was disconnected and locked out. An employee suffered multiple injuries to the right hand from contact with the right hogger blade when attempting to remove a jammed door from the DET machine's first pass section.

29 CFR  1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  On or about June 28, 2021, the employer had not trained each authorized employee on the methods to ensure all residual energy had been dissipated from the Double End Tenorer machine prior to initiating servicing and/or maintenance activities.

29 CFR  1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:   On or about June 28, 2021, on the Double End Tenorer (DET), an employee was exposed to machine hazards associated with rotating cutting blades while attempting to unjam a door. The employer did not ensure that employees implemented energy control application steps prior to unjamming the DET. The machine was not deenergized prior to the employee engaging in the servicing/maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], LOTO device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from machine servicing hazards.  Eggers Industries, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1196154, citation number 1, item 1a and was affirmed as a final order on February 22, 2017, with respect to a workplace located at 164 N. Lake Street, Neenah, WI 54956.
