29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section:   On or about July 6, 2021, the employer did not have energy control procedures to ensure that the bundler unit of a bag manufacturing machine was isolated from pneumatic and electrical energy sources while the assembler was servicing the machine.

29 CFR 1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed: (i) Clear the machine or equipment of tools and materials in accordance with paragraph (e)(1) of this section; (ii) Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; (iii) Remove the lockout or tagout devices as specified in paragraph (e)(3) of this section: (iv) Energize and proceed with testing or positioning; (v) Deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue servicing and/or maintenance:   On July 6, 2021, an employee adjusting the timing on the Bag Machine, Transport, and Bundler of a Holweg/Weber Paper Bag Machine reached inside the Bundler to adjust a stack of bags, without following the necesary steps for testing and positioning of machines, which resulted in a partial amputation of the employee's right thumb. 29 CFR  1904.29(b)(1):What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  On or about July 6, 2021, the employer did not provide the required injury or illness description on the OSHA Form 300 for the calendar year 2021.  Section F of the OSHA Form 300 requires a description of the injury or illness, parts of the body affected, and the object/substance that directly injured or made the person ill.  On June 29, 2021 an employee was injured and Section F of OSHA's Form 300 listed the injury as "Amputation - top of right thumb" without a description of what caused the employee's injury.

29 CFR  1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about July 6, 2021, the employer could not verify that a workplace hazard assessment has been performed.  The employer had assessed the workplace and determined that close toed shoes, eye protection, and respiratory protection were necessary for the safety of employees, but could not provide a signed, written certification of the assessment.
