NY An employee was standing on an unprotected shelve while trying to repair a hoist mechanism that was attached to an I-beam. The unprotected shelve was approximately 12 feet above the concrete ground. Guardrails or personal fall arrest systems were not provided.

On or about 02/22/21, shelve in the warehouse at 28-14 120th Street, Flushing. NY An employee repairing bolts in the hoist mechanism on an unprotected shelve was exposed to an approximately 12 ft. fall to the ground below. The employees were not trained to recognize the hazards associated with working on an unprotected surface in the warehouse.
