29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):   a)  At Walmart store #1705, in the Automotive Care Center garage, on or about 6/28/21 : The employer permitted servicing and maintenance activities on a Coats Rim Clamp X series model 70X-AH-3  tire changer without establishing or implementing a lockout/tagout program which includes written energy control procedures, employee training, application of lockout devices, control of stored energy and periodic inspections, to prevent serious injuries from the unexpected energizing, start up or release of stored energy of the equipment.  An employee sustained a fingertip amputation.
