29 CFR  1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  (a) Fourth Level - An employee erecting precast concrete double tees at a leading edge used a Guardian Velocity self-retracting lifeline as part of a personal fall arrest system.  The labeling and markings, including the model, serial number, and safety instructions, on this self-retracting life had become illegible due to wear, and this personal fall arrest component did not get removed from service, exposing the employee to a fall hazard, on or about June 24, 2021.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1926.501(b)(12): Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who was 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems, unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:  (a) Fourth Level - Employees worked with a crane to set precast concrete double tees on the fourth level above the ground.  The employer's written fall protection plan specified using 100 percent conventional fall protection, to include the use of personal fall arrest systems by employees working at the leading edge while setting double tees.  An employee setting double tees did not follow the employer's written fall protection plan in that this employee disconnected his personal fall arrest system from an anchorage point and worked while not tied off, exposing the employee to falling at least 10 feet to a lower level, on or about June 24, 2021.  No abatement certification or documentation required.

29 CFR  1926.502(g)(1)(i): When control lines were used, they were not erected not less than 6 feet (1.8 m) nor more than 25 feet (7.7 m) from the unprotected or leading edge, except when erecting precast concrete members:  (a) Fourth Level, South East Section of the Building - The employer's written fall protection plan specified using a Control Zone System (i.e., Controlled Access Zone) at leading edge and perimeter areas as an alternative fall protection measure during precast concrete erection related activities.  The employer installed a control line along a 31-foot long unprotected edge in the perimeter at the south west facing side of the south east section of the building, but this control line was less than 6 feet back from the unprotected edge, exposing employees to the hazard of falling from the fourth floor to the ground below, on or about June 24, 2021.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
