29 CFR  1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces were kept in a clean, orderly, and sanitary condition.  On or about July 7, 2021, employees were exposed to slip and trip hazards in the following instances:  a) In the communication's room; there was debris on the floor from the ceiling tiles that had become saturated and crumbling onto the floor.  Boxes of communication equipment were blocking network closets. Trash cans were placed in several areas where rain water was being collected from the leaky roof.   b) Offices had debris on the floor from the ceiling tiles that had become saturated and crumbling onto the floor.  Trash cans and recycle bins were placed thought-out the area to catch the rain water from the leaky roof.  c) The news room floor was saturated with rain water due to a leaky roof.
