On July 1, 2021 at 714 E Cecil Street in Neenah, Wisconsin, the employer did not provide required fall protection safety training for an employee exposed to fall hazards greater than six feet while installing roofing materials.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. a) On July 1, 2021 at 714 E Cecil Street in Neenah, Wisconsin, employees were utilizing an extension ladder on the back of a house to access/egress a roof surface nine and one half feet above surface level without the side rails extended at least three feet above the roof surface. The side rails were extended less than twenty-four inches above the roof surface. b) On July 1, 2021 at 714 E Cecil Street in Neenah, Wisconsin, an employee was utilizing an extension ladder on the front of a house to access/egress a roof surface eleven feet above surface level without the side rails extended at least three feet above the roof surface. The side rails were extended less than twenty-four inches above the roof surface.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On July 1, 2021 at 714 E Cecil Street in Neenah, Wisconsin, an employee was working on residential roof surfaces nine and one half feet and eleven feet above surface level without utilizing conventional fall protection equipment. The Gibson Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1340732, citation number 01, item number 001 and was affirmed as a final order on September 28, 2018 with respect to a workplace located at 2677 W 9th Avenue; Oshkosh, WI 54904.
