Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  a.  On or about July 8, 2021, between aisles R07550 A-D, the warehouse racking had multiple horizontal beams  which were bent inward causing the racking to be unstable to prevent against collapse, thereby exposing employees to struck by hazards.   b. On or about July 8, 2021, between aisles R070076 A-D, the warehouse racking had horizontal beams which were bent inward and beam connections were damaged due to bending and cracking at the bolt holes, thereby exposing employees to struck by hazards.   c.   On or about July 8, 2021, in an aisle adjacent to R075094A, the warehouse racking had horizontal beams which were bent inward and beam connections were damaged due to bending and cracking at the bolt holes, thereby exposing employees to struck by hazards  d.  On or about July 8, 2021, between aisles R080031B-D, the warehouse racking had horizontal beam damage, a bent frame and missing beam connectors causing the racking to be insecure and unstable against sliding or collapse.   e.  On or about July 8, 2021, in numerous areas throughout the warehouse, the beam connections were damaged due to bending and cracking at the bolt holes, thereby exposing employees to struck by hazards.
