29 CFR 1926.453(b)(2)(v): A body belt with a lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about July 8, 2021, on the north side of the site an employee was utilizing a Genie Aerial lift without the use of adequate fall protection as a tethering/restraint system while performing insulating work on the exterior of a building at heights greater than ten (10) feet.
