(a) On or about June 2, 2021 an employee was injured and missed a day of work and the employer did not correctly classify the work related injury correctly. Employer marked column J for "Other Recordable Case" instead of column H "Days Away From Work." (b) On or about June 11, 2021 an employee was injured and missed two days of work and the employer did not correctly classify the work related injury correctly. Employer marked column J for "Other Recordable Case" instead of column H "Days Away From Work." (c) On or about June 29, 2021 an employee was injured and missed three days of work and the employer did not correctly classify the work related injury correctly. Employer marked column J for "Other Recordable Case" instead of column H "Days Away From Work." Abatement Certification and Documentation is required
