29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about July 8, 2021, at the above addressed jobsite, an employee performing residential roofing operations, ten (10) feet two (2) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  b.  On or about July 8, 2021, at the above addressed jobsite, an employee performing residential roofing operations, eleven (11) feet eleven (11) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  c.  On or about July 8, 2021, at the above addressed jobsite, employees performing residential roofing operations, twelve (12) feet ten (10) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  d.  On or about July 8, 2021, at the above addressed jobsite, an employee performing residential roofing operations, nineteen (19) feet seven (7) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  e.  On or about July 8, 2021, at the above addressed jobsite, an employee performing residential roofing operations, twenty-four (24) feet above ground, was not protected from falls, thereby exposing the employee to fall hazards.

On or about July 8, 2021, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification record of the training.
