29 CFR  1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):   a)  At 1824 County Road 49 (Southwest Yard Area): On or about April 28, 2021, the employer required employees to operate a stump grinder with a drive pulley assembly that was not properly guarded; exposing employees to amputation and caught-in hazards.

29 CFR  1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt:  a)  At 1824 County Road 49 (Southwest Yard Area): On or about April 28,  2020, the employer required employees to operate a stump grinder with a drive belt assembly that was not properly guarded; exposing employees to amputation and caught-in hazards.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  a) 1824 County Road 49 (Southwest Yard Area): On or about April 28, 2021, and at times prior,  the employer failed to perform a workplace hazard assessment through written certification for the workplace to determine the hazards and the necessary personal protective equipment for tasks being performed.
