29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  At the construction site located at 4700 N Highland, Building 1601 in Kansas City, Missouri:   Employees were exposed to a fall hazard.  The supported scaffold was not fully planked exposing employees to an incomplete walking/working surface and subsequent fall hazard of up to 17 feet.

29 CFR  1926.451(c)(2):Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  At the construction site located at 4700 N Highland, Building 1601, in Kansas City Missouri:  Employees were exposed to a fall hazard.  The supported scaffold frames were not provided with base plates and mud sills or other adequate foundation.  The fall hazard was greater than 17 feet from the upper working level of the scaffold.

29 CFR  1926.451(c)(3):Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement:  At the job site located at 4700 N Highland, building 1601, in Kansas City, MO, employees were exposed to a fall hazard.  The supported scaffold employees were working from was not plumb to prevent displacement.  The scaffold was built on a hill and not built plumb to the ground exposing employees to a 17 foot fall hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:  At the job site located at 4700 N Highland, building 1601, in Kansas City MO:  Employees were exposed to fall hazard.  Employees engaged in siding work with the use of scaffolding were exposed to a fall hazard of  17 feet from the working platform to the ground below. The employees were exposed in that a complete guardrail system was not in place to keep them from falling.

Employees engaged in siding activities were exposed to injury from a 25 foot fall hazard eave to ground.  Employees were not provided with fall protection while working on installing siding material from the roof as well as window ledges of the building..

Employees using portable ladders to access the roof were exposed to a fall hazard.  The ladder side rails did not extend at least 3 feet above the landing exposing the employees to a fall distance of over 25 feet.
