29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: (a) U.S. Postal Service at 225 S Broadway, Denver, CO 80223: On and before July 8, 2021 the employer did not ensure maintenance employees were protected from injury in that a circular floor hole approximately one-foot in diameter located in the boiler room was not covered. This condition exposed employees to trip hazards.

29 CFR 1910.305(g)(1)(iv)[A]: Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: (a) U.S. Postal Service at 225 S Broadway, Denver, CO 80223: On and before July 8, 2021 the employer used flexible cords to connect electrical power to approximately five fluorescent lighting fixtures that were located on top of casing area workstations. Each lighting fixture was powered at 120 volts and 16.0 amperes. This condition exposed employees to electrical shock and/or fire hazards.
