29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 07/08/2021 in the production area; where an employee was injured while cleaning a glue spill on the conveyor for box line #2 when the machine started up and the employee's hand was drawn into the pinch point between the main drive roller and the black rubber feed roller. The machine was not deenergized and locked in the "off" position. ABATEMENT DOCUMENTATION REQUIRED
