29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.   On or about June 16, 2021 and at times prior thereto, the employer did not ensure employees and employees of subcontractors were protected from falling through a hole in the roof of a building where the metal decking was not secured. The employees and employees of a subcontractor were exposed to a fall greater than six (6) feet to the ground below.
