29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about July 12, 2021 and times prior thereto, employee(s) engaging in roof repair activities on a roof were not protected from falling by the use of  a combination of guardrail systems, safety net systems, warning line system or personal fall arrest system and exposed to fall hazards due to an unprotected edge that measured 14 feet and 10 inches above the walking surface.

29 CFR  1926.502(f)(2):Warning line systems did not consist of ropes, wires, or chains, and supporting stanchions erected in accordance with (f)(2)(i) through (f)(2)(v) of this section.  On or about July 12, 2021 and times prior there to, the employer did not ensure that while workers were present and working on the flat roof the warning line system was complete, roped off and flagged in order to alert workers they were nearing the unprotected edge of the roof.

29 CFR  1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.   On or about July 12, 2021 and times prior there to, the employer did not ensure the portable extension ladder used by employees on site was placed in a location that was not exposed to accidental displacement by vehicle traffic nor was a barricade provided to prevent a vehicle from striking and dislodging the ladder while employee(s) were utilizing the ladder.
