29 CFR  1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:  On or about July 12, 2021, employees engaged in commercial construction operations were exposed to fall hazards in excess of 10-ft. for using an attachment (job made platform) supported by a Genei GTH-5519 all-terrain forklift to work at an elevated level; the platform was not designed or approved by the manufacturer.

On or about July 12, 2021, employees engaged in commercial construction operations were exposed to a fall hazards in excess of 10-ft. due to inadequacies in fall protection training.
