29 CFR 1910.303(g)(1)(ii): The working space at electric equipment rated 600 volts, nominal, or less to ground, was used for storage and a minimum clearance of 3 feet was not maintained: a)Mixing Area: The working space in front of the electrical panel powering the hydraulics that operate the Hess 1500 was blocked by 55 gallon drums and not kept clear by a minimum of 3 feet, on or about 7/12/21. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.147(c)(6):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard were being followed. a)Pavestone LLC: On or about 7/12/21, procedures used by maintenance employees and machine operators to disconnect and control energy sources on equipment, such as but not limited to the mixer, were not reviewed at least annually. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
