29 CFR  1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy, or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  Jobsite: The employer, did not erect falling object protection or secure bundles of roof shingles and other roofing materials away from the edge of the roof thus exposing the employees working on elevated scaffold platforms below to falling objects.

29 CFR  1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end.   Jobsite, rear of garage addition: The employee, working on the elevated platform of the ladder jack scaffold, was exposed to falls posed by the cantilevered end, at the left side of the platform, extending 24-inches beyond the support.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Jobsite, Rear of garage addition:  The employee, working from the elevated platform of the carpenter bracket scaffold, was not protected from falling approximately 15-ft to grade.
