Keefe Ave., Milwaukee, WI, the employer did not ensure that walking-working surfaces throughout the facility including, but not limited to, the molding department, warehouse area and boiler room were free of water accumulation due to leaks in the roof system, thereby exposing workers to slip and fall hazards. (b) On or about August 4, 2021, at the worksite located at 714 E. Keefe Ave, Milwaukee WI, the employer did not ensure hydraulic oil around areas including, but not limited to, aisleways, exits, and molding machines were promptly cleaned, thereby exposing workers to slip and fall hazards. (c) On or about August 4, 2021, at the worksite located at 714 E. Keefe Ave, Milwaukee WI, the employer did not ensure that a floor conveyor system was adequately protected from causing entanglement and trip hazards for workers.
