29 CFR  1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  (a) On or about August 4, 2021, at the worksite located at 714 E. Keefe Ave., Milwaukee, WI, the employer did not ensure that walking-working surfaces throughout the facility including, but not limited to, the molding department, warehouse area and boiler room were free of water accumulation due to leaks in the roof system, thereby exposing workers to slip and fall hazards.  (b) On or about August 4, 2021, at the worksite located at 714 E. Keefe Ave, Milwaukee WI, the employer did not ensure hydraulic oil around areas including, but not limited to, aisleways, exits, and molding machines were promptly cleaned, thereby exposing workers to slip and fall hazards.  (c)	On or about August 4, 2021, at the worksite located at 714 E. Keefe Ave, Milwaukee WI, the employer did not ensure that a floor conveyor system was adequately protected from causing entanglement and trip hazards for workers.

In the event workers exited the building through a route that led to the employee parking lot, they could be contained in a confined area, thus, limiting the potential for adequate distance from the building in the event of a fire or other disaster.
