This was most recently documented on or about July 13, 2021 at U.S. Engineering Services jobsite located at 801 E. Douglas Avenue Wichita, Kansas. Where employees engaged in HVAC component replacement activities were exposed to electrocution hazards in that the employer had not ensured lockout or tagout devices were being affixed to energy isolation devices to control potentially hazardous energy. HVAC unit in which the employees were engaged in component replacement, was found to be energized at 460 volts and 40 amps and employees continued to work in close proximity to energized parts without the utilization of lockout or tagout devices being affixed to each energy isolating device.
