29 CFR 1904.29(b)(3): The employer failed to enter each recordable injury or illness on the OSHA 300 Log within seven (7) calendar days of receiving information that a recordable injury or illness had occurred. On or about July 14, 2021, the Reineke Honda facility at 1033 Bright Road, Findlay, Ohio did not record a recordable injury in the OSHA 300 Logs within 7 days of first learning about the incident, instead entering the recordable injury into the 300 Logs 32 days after the employer received information about the incident.

29 CFR 1904.39(a)(2): The employer failed to report an amputation to OSHA within twenty-four (24) hours after the amputation of an employee's finger as a result of a work-related incident. On or about July 14, 2021, The Reineke Honda facility located at 1033 Bright Road, Findlay, Ohio did not report an employee amputation within 24 hours after first learning that the employee's finger was amputated as a result of a work-related incident. Instead the employer reported the amputation approximately three months after first learning about the in-patient hospitalization.
