29 CFR  1910.217(c)(1)(i):  It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10.  Sheet Metal Dept. - Rockford Systems, Mechanical Power Press #55 (SN BD401B).  On or about June 28, 2021, an employee operated the press with a foot pedal control while placing pieces of foam insulation material into the die area with both hands for the purpose of cutting out pieces of insulation used in the manufacturing of grills.  The employer did not ensure that adequate point of operation guards were provided and applied during setup and used to protect the employee(s) during the operation.  The press was not equipped with adequate point of operation guards (e.g. interlocked barrier guards, 2-hand controls or light curtain, etc.)  to protect employees when a foot pedal control was used to operate the press.   Employees were exposed to amputation hazards.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.217(f)(2): Supervision to ensure that correct operating procedures were being followed on mechanical power press(es) was inadequate:  Sheet Metal Dept. - Rockford Systems, Mechanical Power Press #55 (SN BD401B).  On or about June 28, 2021, an employee operated the press with a foot pedal control while placing pieces of foam insulation material into the die area with both hands for the purpose of cutting out pieces of insulation used in the manufacturing of grills.  The employer did not ensure adequate supervision of employees who operated mechanical power presses to ensure that proper and safe operating procedures were being followed.  Employees were exposed to amputation hazards due to machine guarding deficiencies and the placement of body parts into the danger zone (die area) of the press.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
