29 CFR  1910.502(c)(1):The employer did not develop and implement a COVID-19 plan for each workplace with all the required site specific information.  a) Embassy of Ivy Hill, located at 1401 Ivy Hill Rd, Wyndmoor, PA 19150: On or about July 15,2021, the employer failed to develop and implement a COVID-19 plan to mitigate the spread of SARS-CoV-2, the virus that causes Coronavirus Disease 2019 (COVID-19). Employees working in the first and second floor having the potential to provide care to suspected and/or confirmed COVID-19 residents, were not provided with the required site-specific information as specified in this section.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR  1910.502(c)(2): The employer's COVID-19 plan was not in writing.   a) Embassy of Ivy Hill, located at 1401 Ivy Hill Rd, Wyndmoor, PA 19150: On or about July 15,2021, the employer whose employees were potentially exposed to SARS-CoV-2, the virus that causes Coronavirus Disease 2019 (COVID-19), did not have a written COVID-19 plan.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR  1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a)  Embassy of Ivy Hill:  On or about July 15, 2021, the employer did not develop and implement a written respiratory protection program with worksite-specific procedures for respirator use that included all provisions in 29 CFR 1910.134(c)(1)(i) - (ix).  Ivy Hill Rehab LLC, dba AristaCare@ Ivy Hill was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.134(c)(1), which was contained in OSHA inspection number 1475891, citation number 1, item number 1a and was affirmed as a final order on February 10, 2021, with respect to a workplace located at 1401 Ivy Hill Road, Wyndmoor, PA 19150.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
